With each of these reviews I'll include 2 cropped caps from the episode. One of Dean and one of Sam. I decided to do something different and make them b&w.

I'm trying to get this posted as soon after the episode as possible, again by using caps made from my recording of the episode. Hopefully there's still some squee left out there. ;)
Since this was an especially intense and emotional episode, I didn't take any notes until my 2nd viewing and filled them out a bit on my 3rd.

Recap: Lilith, Anna, Mary, demon blood, Sam not wanting to be a freak, Sam's blood sucking, Dean finding out, and locking Sam up.
Love the red light on Sam when he's talking to Dean through the door. Nice touch.
Sam denies he's a junkie and Dean's not buying.
He's getting strong to kill Lilith....Dean says he's weak, desperate, pathetic.
Sam accuses him of being self-righteous....ouch. Boys, stop this....
“Bench-warmer seat to the Apocalypse” Heehee! Dean.
Love the look on Dean's face as he climbs the stairs with Sam calling after him...oh boys...
Sam's not feeling so good....
Cold spot! Lights flickering....Alastair! Eeek!
Oh god....this is tough to watch....Sam pleading for Alastair to not hurt him....*cries*
What an awful hallucination....Sammy!
And poor Dean & Bobby hearing him scream....

Rufus! On the phone....'course we never hear his voice....bummer.

Castiel meets Dean outside....and took his time getting there, sheesh....2 hours!!!
He can't/won't tell Dean what was all important that he had to tell from the last eppy.
Uh oh, if Sam drinks enough demon blood to kill Lilith, he'll change forever into something Dean would want to kill...
Cas wants Dean to step up and take his role.....take Sam's place in a way...
“If I do this...Sammy doesn't have to?” Awww....I love big brother Dean! *hugs him*
and Dean accepts, but notice he doesn't quote the oath perfectly....he says “you guys” instead of “angels”. You gotta wonder if that'll be a loop hole he'll use later. Heh. ;)

“I would die for him in a second. But I won't let him do this to himself. I can't. I guess I found my line. I won't let my brother turn into a monster.” Awww, Dean. I love you!
“You're nothing to me.” Ouch! Sam, don't believe that!
And he's alone again....
Time passes....and the handcuffs are opening.....WTH???
And the door opens....oh no
Is this a hallucination?.....Guess not....

Dean and Bobby are sleeping....awww.
Cas looks bad, defeated....
Anna! “What did you do?” Good question!
He was following orders? WTH????
Uh, oh, she's captured and spirited away. That can't be good.

He knows Bobby won't shoot him....oh, man....now they're both getting close to tears...*wibbles*
So Sammy knocks Bobby out with the gun and drives off....oh, Sammy....no, no, no...

Dean & Bobby try to figure out how Sam escaped....they're guessing Ruby, but how?
Dean hopes Sam's with Ruby 'cause he's gonna go kill her. Woot! Go Dean!
Sam's got the shakes....sitting in a motel room....
Ruby! Heh....the honeymoon suite!
She's glad to see him, but sorry he's hurting (yeah, right)....and Sam's upset about what Dean did to him....knows Dean will know how to find him....
Heh.....yeah, enough talk Ruby, Sammy's hungry! :P
Yeah, she's so smiling like she's getting exactly what she wants.....oooooh
Did Sam mess up the Impala to slow up Dean? Ooooh...
Bobby's tracked the car Sam switched to.....and Dean's onto Sam's tricky ways... ;)
Ruby says his appetite's gotten bigger, and the seals are almost all broken....
Seal 66 can only be broken by Lilith....so this convinces Sam that if he can kill Lilith it stops all of this. Don't believe her Sammy!
Ruby knows of Lilith's personal chef....a lead to get close to her....
So Lilith likes to eat babies....nice... Kinda backs up Lilith's line of wanting things to go back to being “all baby blood all the time” from an earlier eppy. :P

Bobby comes through again....Cold Spring is lighting up with demon signs....reminds Dean to be good to Sam, to get through to him....
Sam leaves the room....and Dean enters....heh
He attacks Ruby, but Sam stops him...
Sam wants to talk....Ruby runs out....now it's just the boys
“Sammy you're lyin' to yourself. I just want you to be okay! You would do the same for me, you know you would.”
Sam's got all the answers...or so he thinks...
Dean's willing to go with Sam to go after Lilith, but not with Ruby....Sam says it's the only way, that he needs Ruby to help him kill Lilith
Sam's determined to do this himself....he's doing what needs to be done because Dean can't...
He asks Dean to trust him....but Dean says no....Sam's too far gone....it's not what he's doing, it's what he is...a monster....Oh, God, Dean's single tear.....oh, boys!
It's fight time....

Dean: “You walk out that door, don't you ever come back!” Oh, man....the music, too....the theme...
Oh, Sammy! Why....
Wow. Can you say heartbroken? The final division between the brothers has fallen. It's easy to take sides here, as the episode pits the brothers against each other, but I'm guessing most sympathies will go with Dean this time. If you look at the episode as a whole, Dean is the one making the sacrifices, as he always has, for Sam. Every action he makes here is for Sam, to help him. On the other side, Sam's motivation is pretty self-centered, despite his belief that he's doing this for the greatest of reasons....to save the world from the Apocalypse (with a side of revenge thrown in). The problem is that he's so far from being himself that he's hurting Dean. He's believing everything Ruby feeds to him. He also expects Dean to believe and trust him, but after all of his lying, how can he? Dean's been incredibly patient, trying to find answers and a way to get through to Sam, but has he truly lost him? I hope not. The turning point was at the end of the fight when Sam tried to strangle Dean. Something stopped him (thank God), but is there enough of Sam left in there for Dean to reach? I have some hope that there is. It truly hurts to have them so broken apart like this.
Then we have the surprise of the eppy in the form of Castiel. What on earth (or heaven) is going on with our favorite angel? He seemed truly upset over asking Dean to pledge himself to God & the angels, yet he released Sam and let Anna be captured. Is he being controlled by some other rogue angels? His actions point to him switching sides, but it's hard for me to believe that. There has to be an explanation and I hope we'll get one next week.
Theory time: However, in my sick/twisted brain I'm also wondering if Cas isn't really a heavenly angel after all. What if he's really working for Lucifer? A dark angel perhaps? What if the brothers are being pitted against each other in this demon war that was talked about in S3? There's Lilith's side and the other camp. I dunno...I'm just confused by Castiel's actions and I'm not really liking the way things are headed.
The episode delivered in all areas (minor pet peeve though....no CU shots of the guys! Grrrr). Jensen & Jared's acting was superb. I bet they had a great time filming that fight scene, too. Awesome work there. I knew this episode would be upsetting and emotional and it was. Kripke, you bastard! Fix my boys! I really don't want to have to live with them like this over the summer. It'll be agonizing. Tell me there's some way to fix this, some hope out there that Sam will see the error of his ways!
I might think of other things to say later on, but for now this is all my befuddled brain can muster up. The boys sold it so well and I wept (internally) for the brothers. Can it be next Thursday now?
